Out-Patient Availment Procedure

Things needed:

· HMO Card

· Valid Government ID or Company ID

Step 1:

Begin by verifying if the hospital is accredited with your HMO card.

Step 2:

Check the list of accredited doctors and facility for each HMO by clicking below, if uncertain about the type of doctor needed, proceed to the hospital's HMO section or proceed with the HMO Coordinator in the hospital.

Step 3:

Request a Letter of Authority (LOA) through the hospital's HMO section, HMO Coordinator, HMO Mobile App/Website, or contact your Servare Health Account Officer for LOA assistance. If you have a existing/preferred doctor, you can request an advance LOA (at least 3 days before your appointment). You can also check the doctor's schedule in advance through the list or the hospital's website.

Step 4:

Attend your doctor's consultation. If the doctor prescribes laboratory tests or other diagnostic procedures after the consultation, repeat Step 3 to obtain an LOA for the tests. Ensure the doctor's prescription states the request, including the probable diagnosis.

Step 5:

Present the Letter of Authority (LOA) and your HMO card to the hospital/clinic test area. Proceed with the recommended tests. If the consultation and tests lead to hospital confinement, follow the In-patient guide availment.
