In-Patient Availment Procedure

Things needed:

· Accredited Doctor’s Admission Order

· HMO Card

· Valid Government ID

Step 1:

First, verify if the hospital is accredited with your HMO card.

Step 2:

If the hospital is accredited, proceed to the hospital's admission section. If not, visit an accredited hospital to prevent cash-out payments and denied claims. You can find the list of accredited facilities for each HMO by clicking below:

Step 3: Fill out the admission form and inform the hospital representative that you're availing services through your HMO. Present your HMO Card along with the Admission Order. The hospital representative will initiate a call to the HMO for Letter of Authorization (LOA). Afterward, the representative will discuss your eligibility, Maximum Benefit Limit, Room & Board.

Step 4: Proceed with the hospital admission process. An HMO Roving Liaison Officer (RLO) will contact you within 24-48 hours of admission. The RLO will assist you with the requirements needed during discharge.
